To Fill Out the Form:
- Choose Color and if you want the optional slider lid or the regular lid.
- Enter your name or up to 3 initials
- Choose either a name or monogram font
- If you don't want it personalized put N/A in the name box and we'll engrave the design on both sides. (or only one - please let us know in the additional info box)
- If you want your name within the letter like our Madelene sample please choose Times Font and in the additional info box state "Place Name Within Initial"
